While a decrease in high frequency EEG activity is associated with a decrease in arousal in drug-free conditions, sedative benzodiazepines increase beta activity. However, the beta and gamma increase was specific for active behavior and not for inactivity. Non-benzodiazepine GABA(A) receptor modulators can increase beta activity as well. A pharmacological dissociation of the relation between electroencephalographic (EEG) activity and behavior has been described for the benzodiazepines. Both drugs modulate the GABA(A) receptor, albeit that zolpidem shows alpha(1) subunit selectivity while diazepam is non-selective. Interestingly, diazepam and zolpidem seemed to amplify, rather than dissociate, the relation between behavior and the EEG. It is hypothesized that the large increase in beta-3/gamma activity caused by diazepam and zolpidem is a compensatory mechanism that allows for behavioral activation, despite pharmacologically induced sedation. A detailed analysis of rat open field behavior was made with a distinction of 25 behavioral elements. To further study the relationship between rat behavior and EEG under GABA(A) receptor modulation, EEG effects of diazepam (2.5 mg/kg) and zolpidem (2.5 mg/kg) were studied during different behaviors. Both diazepam and zolpidem increased EEG beta frequencies, characteristic for the benzodiazepines. The EEG was segmented according to each behavioral element and a corresponding power spectrum calculated.

Subjects completed mood questionnaires before and at regular intervals after taking the drug, and performed a vigilance task during the 60-minute period of tracer uptake. Each subject was tested three times, at 1-week intervals, with placebo, a low oral dose of diazepam (0.07 mg/kg), and a moderate dose of diazepam (0.14 mg/kg). The changes in metabolic rate were not clearly related to other observable drug effects. The effects of diazepam on regional cerebral metabolism were examined in eight healthy volunteers using positron emission tomography with 18-fluorodeoxyglucose as the tracer. Thus, diazepam decreased whole brain metabolic rate at doses that produced only modest subjective or behavioral effects. Both doses of diazepam decreased global (whole brain) metabolic rate but did not affect specific regions differentially. Subjects experienced sedative like effects during all three scans (placebo as well as drug). The effects of the drug on cerebral metabolism were examined alone and in relation to the subjective and behavioral effects of the drug. Compared to placebo, both doses of diazepam decreased anxiety, and neither dose produced significant impairment of task performance. Neither the subjective nor behavioral drug effects were correlated with the changes in metabolic rate.

However, dosage seemed to be inversely related to the degree of memory impairment. Ten patients who were prescribed daily doses of diazepam for the treatment of anxiety, insomnia, or psychosomatic symptoms were assessed repeatedly on measures of short-term and long-term memory on and off the drug. Both kinds of memory, especially the latter, appeared detrimentally affected by the drug. The negative relationship between drug dosage and degree of memory impairment might be due to habituation toward this particular side effect by heavier drug users. It is suggested that on theoretical grounds, the use of diazepam in conjunction with the behavioral psychotherapies may be contraindicated. In general, the results are consistent with the hypothesis that diazepam interferes with the memory consolidation process.
