Diazepam undergoes N-demethylation to nordiazepam, a metabolite at least as active as its parent. Both of these compounds are converted to 3-hydroxy.

Please note, this assay is for the quantitative determination of four benzodiazepines (diazepam, nordiazepam, temazepam and oxazepam) in blood.

Over a 2-year period, we found 94 cases of DUID in which the concentrations of diazepam in blood was 1.1 μg/g or more. Diazepam (D) and nordiazepam (ND).

D and ND were the only drugs present in eight cases (seven men and one women) and in another five cases ethanol was present at concentrations ranging from 0.81 to 1.98 mg/g. Over a 2-year period, we found 94 cases of DUID in which the concentrations of diazepam in blood was 1.1 μg/g or more. The concentration of D in blood exceeded 2 μg/g in 21% of cases and was over 3.0 μg/g in 11% of cases. Other psychoactive prescription drugs were identified in blood including alprazolam, flunitrazepam, oxazepam, zolpidem and zopiclone.

Preparation of Nordiazepam. by Psycho Chemist. 2-(chloroacetamido)-5-chlorobenzophenone. In a 500 mL-round bottom flask, dissolve 46 gr.

Add a few crystals (200 mg) para-toluenesulfonic acid and 250 mL toluene, and reflux with stirring (Dean-Stark-adapter, double-surface condenser) with azeotropic removal of water for 2 hrs. of refluxing (all should be dissolved now), distill off any volatiles in vacuum. Let cool down, the next morning, the flask is shaken vigorously to break lumps. After re-cooling of the mixture to 70C, add 150 mL hot (75C) water with stirring. (Use a safety gas washing bottle between bubbling and generating unit!).

The brain-plasma distribution of total ND and oxazepam (OX) is about equal in ND dependent dogs but in DZ dependent dogs total ND and OX are about 2-fold higher in brain than in plasma. The levels of DZ, ND, and OX in the extraneuronal brain space are similar to their unbound levels in plasma. Microdialysis probes were also used to determine the unbound parent drugs and their metabolites in plasma. These data suggest that the concentration of free benzodiazepines in plasma is a good approximation of the concentration in the vicinity of the membrane receptors in the dependent dogs. The levels of DZ, and ND and their metabolites were determined during the last week of stabilization in the extraneuronal brain space, in brain tissue, in plasma and in CSF. In these studies dependent dogs were anesthetized with pentobarbital and microdialysis probes were inserted bilaterally into the parietal cortex and perfused with artificial cerebrospinal fluid. The compartmental distribution of diazepam (DZ) and nordiazepam (ND) and their metabolites was studied in DZ and ND dependent dogs.
